It wasn't until 1959 that Syntex succeeded in creating Megestrol Acetate. Megestrol Acetate was derived from medroxyprogesterone acetate, which was synthesized in 1957 at Syntex. Megestrol Acetate was the third synthetic progesterone derivative developed for medical use, after hydroxyprogesterone caproate in 1954 and medroxyprogesterone acetate in 1957. In 1951, progesterone was discovered to be effective in treating endometrial hyperplasia, and in 1959, progestins were discovered to be effective in treating endometrial cancer.

In the 1980s, clinical studies of very high doses of Megestrol Acetate for breast cancer observed a marked increase in appetite and weight gain in patients despite their advanced cancer. An article published in 1986 suggested that Megestrol Acetate be studied and potentially used in cachexia, and this sparked interest in it as an appetite stimulant.
